Release Form

I, acknowledging I am the parent or guardian of the child(ren) listed above, hereby agree to release, discharge, indemnify and hold harmless the Rambler Wrestling Club, including but not limited to its Board of Directors, coaches, assistant coaches, volunteers, employees, and representatives, from any and all rights, claims, and/or causes of action that may arise as a result of my child(ren)'s participation in club activities, practices, or events.

I have read this release, waiver, and indemnity agreement and fully understand its terms. I voluntarily sign it of my own free will.

Medical Consent

In the event reasonable attempts to contact me have been unsuccessful, I hereby give my consent for the administration of any treatment deemed necessary by the above-mentioned doctors or, if unavailable, by another licensed physician or dentist. I also authorize the transfer of my child(ren) to any hospital reasonably accessible.

This authorization does not cover major surgery unless the opinions of two other licensed physicians or dentists concur in the necessity for such surgery.

I give permission for medical information to be shared with wrestling personnel who interact with my child(ren).

Membership Directory

To improve communication within the organization, we will create a 2026-2027 Membership Directory including wrestler name, parent name(s), contact phone(s), and email address(es). Texting may be used for immediate communication. Carrier fees may apply.

Media Release

I hereby grant the Rambler Wrestling Club permission to use photographs, video, or audio of my wrestler(s) listed above for promotional and educational purposes.

I waive all rights and release the club from any claims related to the use and distribution of this media.

Code of Conduct Agreement

Achieving a high level of sportsmanship in our sport starts with YOU. Participating in wrestling is a privilege, not a right. As a representative of our community, our wrestling family, and the sport, you are expected to conduct yourself in a way that reflects positively on yourself, your coaches, teammates, family, and the Rambler Wrestling Club.

By signing below, I agree to the following:

• I will always show good sportsmanship.
• I will respect and not criticize my teammates, coaches, or others.
• I will control my temper.
• I will respect the opposing team and shake hands at the beginning and conclusion of each match or dual.
• I will not taunt or yell at opposing players or teams.
• I will respect referees and not openly criticize them.
• I will show respect to all equipment, supplies, and facilities.
• I will not make any alterations to club-issued apparel or equipment.
• I will strive to have fun and help ensure my teammates have a fun wrestling experience.
• I will always play hard and fair.
• I will do my best to be on time and attend practice regularly.

I understand that any violation of this Code of Conduct may result in disciplinary action, including suspension or removal from the club, team, or events, without refund. Concerns should be brought to the attention of a coach, board member, or adult immediately. The club reserves the right to handle situations individually and as they see fit.
